Complete and submit the Eye Donation Eligibility Assessment Checklist (EDEAC):

Call the number above and leave a voicemail message with the patient's name, hospice name, location and contact details.

This voicemail is monitored between 8am to 8pm, 7 days a week. Outside of these hours you will be contacted after 8am the following day.

A specialist nurse will guide you through the referral process and the information required.

Please have to hand the following:

• Patient demographics
• Details of preferred contact? (Family member or next of kin)
• Time of death (Eye Donation can only take place within 24 hours of death)
• Reason for admission (Medical condition which required admittance to hospice)
• Past medical history
• Provisional cause of death (if known)
• Has a discussion taken place with a Medical Examiner? (to the best of your knowledge)
• Is the case likely to need referral to the Coroner? (to the best of your knowledge)
• If the case needs referral to the Coroner, check this referral has been made

The team at the National Referral Centre will then explore the possibility of donation.
